1. You can request for Transferring Professional, if the facility license is:
-Active
-Expired and the expiry date is not expired more than 6 months
2. Attach No Objection Certificate for transferring license to another health facility.
3. A. G. Practitioner: Not allowed to transfer from Hospitals, Daycare Centers, Hotels, and Schools to any other types of Healthcare facilities.
B. G.P. (Specialty): Not allowed to transfer from Hospital, Daycare Centre to any other types of Healthcare facilities
